Applications are invited from suitably qualified candidates for admission in to a twelve 12, month training programme for intensive Care Nurses for 2017/2018 academie session.

Applicants raw, ressterzd nurses with at least two (2) years post registration working experience with up to date practicing license.
Method of Application:
Forms are obtainable in the office of the Chairman. Medical Advisory Committee at the JUT, Permanent Site, after the payment of non refundable fee of N7,500.00 (seven thousand, Site hundred naira) only to the hospital account through Remita, http://remita.net payable to University Teaching Hospital Jos and click on sale of form/School fees for ICN School admission form.
Submission of Forms:
All the completed application forms are to be submitted in a big brown envelope to the office of the Course Coordinator on or before 21st April 2017 at JUTH permanent Site.

1. The form can be obtained at either the College Premises or Ministry of Health Kaduna and it is FREE.
* Date of Collection: 02 - 10/02/2017.
* Time: 8:00am- 3:00pm daily.

HOW TO APPLY
1. The application form is obtainable at the school complex or can be filled online at www.uath.gov.ng
2. For online filling of the admission form; Pay a non-refundable application fee of SIX THOUSAND NAIRA (N6.000) using the process stated below:
TO PAY FOR THE FORM ONLINE:
- GO TO www.remita.net.
- Click on PAY GOVERNMENT AGENCY.
- Name of MDA is U.A.T.H.
- Name of service is IGR (Internally Generated Revenue).
- Input Applicant's full name, email and phone number.
- Confirm transaction and proceed to payment.
- Copy the RRR number that will be displayed take to any commercial bank and pay for the form.
- Scan the remita receipt / print out from the bank and bank teller together.
Proceed to www.uath.gov.ng to fill the application form. (Closing date for the purchase of the form is on the morning of the examination date).

2. Go to www.uath.gov.ng, scroll down the page and click on application form
3. Then click on register now, read the instructions carefully and continue with the registration
4. Fill the Application Form online and submit
5. Print out the duly filled Application Form.
6. For editing and reprinting of your form, go to the login page, input applicant's email and password and continue.
7. Come along with printed application form and the bank teller with the remita receipt used for payment on the entrance examination day.
